Please consider my application for membership in OTTAWA VALLEY AID FOR CHERNOBYL CHILDREN (OVACC).

The security proceedures and policies will include:

A home-visit for first time families with an interview by a committee of a least two OVACC representatives. All members of the household must be in attendance. Three references are required from people who are not relatives. One reference must be from a neighbour. A criminal record check will required for all members of the household who are over 18. You are not permitted at any time to cross the US boder with a child from Belarus. You must submit a signed copy of a competed application at the time of the home visit.
